Senior Citizen Medical I.D. Program

The free Chicago Senior Citizen Medical I.D. Program is a service provided by the Office of the City Clerk.

To get an I.D., seniors fill out information on an application form and have a digital photograph taken. The senior's photograph and information will be securely stored by the city clerk's office. The information will be readily available to law enforcement agencies and medical professionals for the purpose of identifying the named person if they become incoherent or unconscious.

The I.D. card will include the following information:

  • The individual's picture
  • Emergency Contact
  • Address
  • Contact Number
  • City/State
  • Medical Conditions
  • Birth Date
  • Medical Alllergies
  • Doctor
  • Blood Pressure
  • Doctor's Number
  • Insurance Company

The medical I.D. card will then be mailed to the individual's place of residence 2-3 weeks following the appointment. This service is free to all senior citizens ages 60 and over. To schedule an appointment or to receive more information, please call the city clerk's office at 312-744-2506.
